KANAWHA COUNTY, W.Va. (WSAZ) -- A combination of human error and extremely cold temperatures are being blamed for a slurry spill in eastern Kanawha County.

Investigators told lawmakers the valve at the Kanawha Eagle Prep Plant that failed and allowed the slurry to leak was last closed manually.

A build up of ice prevented it from closing all the way, so when it was reactivated, it started leaking and then the seal failed completely.

More than 100,000 gallons of slurry spilled into Fields Creek and the Kanawha River last week.

So far tests show the water quality downstream has not been impacted.
